Step 1: Booking Your ADHD Assessment
To get started, contact us directly to arrange your ADHD assessment appointment. We offer flexible scheduling Monday to Saturday, with remote, face-to-face, and school/home-based assessments available (the latter for school-aged children, by prior arrangement).

Once your appointment slot has been confirmed, you will receive a secure link to complete your booking and make full payment of ยฃ789. Payment is made via Stripe, which accepts all major cards and offers flexible instalment options through Klarna (subject to eligibility).

Once scheduled, we will send you an initial medical and psychosocial questionnaire to collect essential details, including:

โ๏ธ Supporting documentation โ If you have educational psychology reports, workplace assessments, or other relevant paperwork, you will be asked to send them in advance or bring them to your appointment.
โ๏ธ Summary Care Record submission โ You will provide a copy of your medical records, ensuring we have a full history of treatments, diagnoses and previous care interactions.
Step 2: Collecting Additional Records & Screening Tools
Before your full ADHD assessment, we conduct targeted pre-assessment screening to ensure an accurate and thorough diagnostic process. We select tailored tools based on your history, which may include:
๐ SNAP-IV โ A well-established questionnaire measuring ADHD traits in children and adults.
๐ Kessler Psychological Distress Scale (K10) โ Assesses mental health distress, helping differentiate ADHD symptoms from anxiety or depression.
๐ Risk Questionnaires โ Screening for suicidal ideation, self-harm risks and safety concerns, ensuring a full mental health review.
๐ Substance Misuse Screening โ Determines whether ADHD symptoms contribute to self-medication or substance use behaviours.
๐ Informant Questionnaires โ Reports completed by parents, partners, teachers, or colleagues, offering an external perspective on ADHD-related challenges.
๐ Executive Function Assessments โ Evaluating working memory, impulse control, focus, and organisation skills.
These pre-assessment tools, combined with the Summary Care Record, provide critical insights before the full ADHD evaluation, ensuring we tailor the assessment to your specific experiences.
Step 3: Completing the QB Check
In addition to standard screening tools, you will complete a QB Check, a computer-based test designed to scientifically measure ADHD-related cognitive patterns.

Step 4: Full ADHD Assessment โ DIVA-5, Young DIVA, or DIVA-LD
Your two-hour ADHD assessment is where we explore your symptoms, challenges, and history in depth, conduct a physical examination and discuss your medical, social and psychological history, using face-to-face or virtual structured diagnostic interviews, including:
๐ DIVA-5 (Diagnostic Interview for ADHD in Adults, Fifth Edition) โ A structured clinical interview based on DSM-5 criteria, assessing ADHD symptoms across childhood and adulthood and their impact on daily life.
๐ Young DIVA โ A specialised child assessment designed to evaluate ADHD traits in younger individuals while distinguishing symptoms from typical childhood behaviours.
๐ DIVA-LD โ A tailored version of DIVA for individuals with learning difficulties, ensuring comprehensive evaluation while accounting for cognitive differences.
This structured clinical assessment ensures we go beyond screening tools, focusing on your lived experience, struggles, strengths and goals.
Together, we will explore:

๐ก Current difficulties โ Impact on work, relationships and daily life.
๐ Neurodevelopmental history โ Identifying co-existing conditions like autism, dyslexia, or anxiety.

Adult assessments can be conducted face-to-face or virtually, while child assessments take place in-person at RevitaMed Health Private GP Services, Aesthetics, Ormskirk, Southport, or may be arranged virtually, at school, or at home if appropriate. You are welcome to attend alone or bring a family member, friend, or trusted supporter.
Step 5: Diagnosis Outcome & Holistic ADHD Management Plan
At the end of the assessment, we will discuss your diagnosis, ensuring you fully understand how ADHD presents in your life. If needed, your diagnosis may be reviewed by our Multidisciplinary Team (MDT) before final confirmation.
We provide official support letters for schools, workplaces and universities, helping you obtain reasonable adjustments, learning accommodations and workplace modifications.
Your personalised management plan will focus on holistic, non-medication strategies, including:
